Parent income in calendar year 2022 or 2021 is less than the 2020 income reported on the FAFSA and/or CSS Profile
Loss of untaxed income (social security, disability, child support, etc)
One time taxable income (IRA, Pension distribution, back-year child support payments, etc.)
Unreimbursed medical expenses (for immediate family members only)
Death or disability of a parent
Request to extend need-based eligibility beyond 8 semesters (or lower equivalent for transfer students)
Reduction in student academic year earnings due to Covid or other hardship

Termination notice or letter of explanation from employer
Severance documentation
Unemployment benefits documentation
If benefit terminated, provide documentation of monthly benefit amount and date of benefit termination.
If benefits reduced, provide documentation of original amount, date of reduction and reduced amount.
Documentation identifying the source of the one-time taxable income and confirmation that the income was not received in the next year.
Include in the explanations section below a statement of your expected academic year earnings, a dollar amount of the funding that will not be earned, and the specific educational or related expenses you planned for those earnings.
Statement from the insurance provider stipulating amount of medical expenses not covered by insurance (preferred documentation)
Alternatively, you may submit medical bill(s) confirming payment (must exclude amount covered by insurance)
Itemized bills must be tallied for total expenses paid (medical expenses should be greater than 10% of income to be considered)
Death certificate or verification of disability status
Social Security benefits received for all family members
Inheritance documentation
Life insurance documentation
Please enter below (or upload a letter describing) the extenuating circumstances that led to your not completing the degree requirements within eight semesters (or the lower equivalent for transfer students). Please also share with the committee your current plans and timeframe to finish degree requirements.
Please upload a letter describing any other factors you would like the committee to consider in the determination of the amount of assistance offered to the student and provide any documentation to support your request.
Note: we are unable to consider appeals based on circumstances that include but are not limited to: personal expenses (travel, hobbies, leisure), and/or expenses that have not yet occurred.
